    Organizational structure: Mosaic was formed in 2003 by the merger of Bethphage (est. 1913) and Martin Luther Homes (est. 1925). Mosaic provides services in the United States in AZ, CO, KS, NE, TX, IA, IL, WI, IN, CT, and DE.Each service agency is headed by an Executive Director. Agencies are grouped into geographic regions, headed by a Regional Vice President. The Mosaic national headquarters is in Omaha, NE. Mosaic is headed by the Chief Executive Officer who reports to a volunteer Board of Directors. Mosaic's international work is conducted through an informal alliance including Mosaic and Bethesda in the US, Bethel in Germany, and Bethphage in the United Kingdom. All international work is owned and operated by indigenous NGOs and Mosaic's role is to support, train, and provide technical assistance.

    Affiliation with NGO networks: Impact - an international alliance that promotes the rights of people and works for the inclusion of people with disabilities. (Impact includes organizations from the US, UK, and Germany.)
